the KELLYDIRECTORY.COM with two plans offered. The Multi User plan allows
an unlimited number of users online per facility. The fee is only $900.00/year. The Single User plan allows one user online at a time. The fee is $300.00/year. (California

The internet publication includes active links
to the web sites of most payers listed. The user clicks the link and visits the payer web
site. When done the user clicks the back arrow and returns. Cross reference listings are

To order use the order form. Phone, fax and
e-mail orders also are accepted for payment by MasterCard or Visa. Payment
is in advance but orders to be billed are accepted on the condition of payment within 30
days of going online. The plans have a 30 day satisfaction guaranty. There is immediate
access on ordering. The ID/Password is e-mailed when an order is received. Plan stays in
effect until cancelled. All of a member's personnel use the same
ID/Password.
